Lead Palliative Care Physician

Virtus Healthcare Staffing is seeking a specialized Lead Palliative Care Physician to join our partner's team at Monogram Health in Atlanta, GA. This role is ideal for a quality-driven physician eager to apply their clinical expertise in a managed care, population health context, primarily delivering in-home care to seriously ill patients with conditions like CKD/ESRD, COPD, and CHF .

The successful candidate will be an in-market specialist, integrated with the local Market Physician Executive (MPE), focusing on patient quality of life, education, and shared decision-making. This is a pivotal role requiring a deep understanding of palliative care, a commitment to improving healthcare outcomes, and the ability to build strong local provider relationships.


Qualifications

Required:

– Doctorate Degree in Medicine.

– Board Certified in Hospice and Palliative Medicine.

– Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare.

– Willingness to become licensed in multiple states as company and patient needs require.

– Desire to practice in a forward-looking renal care environment focused on population health management.

– Demonstrated experience applying evidence-based clinical criteria.

– Comfort and skill in goals of care discussions and advance care planning.

– Experience with high-need Medicare Advantage and managed Medicaid populations, as well as commercial populations.

– Experience integrating acute and behavioral health care and appreciation of social determinants of health and the value of social workers.

– Expertise in managing medication panels for complex patients.

– Must be willing and able to obtain hospital privileges at required facilities.

– Must have reliable transportation.


Key Responsibilities

Patient Care & Clinical Excellence

– Proactively identify patients who qualify for palliative care services and build a patient panel in collaboration with the MPE and care team.

– Treat a panel of patients primarily in their homes to drive healthcare outcomes.

– Evaluate and treat symptoms that may interfere with quality of life, such as pain, pruritus, depression, anxiety, and insomnia.

– Refer members to appropriate specialists (Monogram-based or locally-aligned) to optimize clinical care.

– Conduct goals of care and family meetings as needed, accurately documenting patient/family wishes in state AD/POLST forms.

– Support patients utilizing the interdisciplinary team (IDT), including the Palliative Care Chaplain, and assist with hospice transitions as appropriate.

Staff Management & Leadership

– Develop and present metrics to executive leadership as requested.

– Be responsible for the outcomes of the patient panel, driving predetermined clinical and financial Key Performance Indicators (KPIs) through interventions.

– Participate in monthly meetings with the Regional Medical Director (RMD)/Chief Medical Officer (CMO) to discuss KPIs and expectations.

– Educate and support the team regarding palliative care, hospice care, advance directives, symptom assessment, and communication.

Provider & Client Engagement

– Develop key relationships with local providers, including PCPs, nephrologists, and other specialists, to improve market alignment.
