Provide Compassionate Care to Seniors

As a Nurse Practitioner in Long-Term Care, you will play a vital role in meeting the healthcare needs of geriatric patients. You will assess, diagnose, and manage a variety of health conditions, working collaboratively with an interdisciplinary team to provide holistic and patient-centered care.

Responsibilities:

• Provide direct patient care, including assessment, diagnosis, treatment, and management of acute and chronic conditions.
• Develop and implement individualized care plans.
• Order and interpret diagnostic tests.
• Prescribe medications as appropriate.
• Educate patients and their families about health conditions, treatment options, and preventative care.
• Maintain accurate and timely medical records.
• Collaborate effectively with physicians, nurses, and other healthcare professionals.
• Participate in quality improvement initiatives.

Qualifications:

• Education: Relevant degree and training to become a Nurse Practitioner.
• Licenses/Certifications:
