Director – Care Management Services

Location: Missoula, MT | Full-Time | Regional Medical Center

Are you a compassionate leader ready to make a meaningful impact in patient care? We're looking for a Director of Care Management Services to join our team at a 151-bed acute care facility, a trusted regional medical center providing essential emergency services to a vast geographic area.

Lead and Innovate in Care Management

In this role, you'll oversee daily operations, develop strategic initiatives, and foster collaborative partnerships to improve patient care coordination, discharge planning, and utilization management. Your leadership will directly impact patient outcomes, satisfaction, and overall care experience.

What You'll Do:

Lead and mentor a team of case managers and healthcare professionals.
Enhance patient care coordination by optimizing discharge planning and utilization management.
Collaborate with interdisciplinary teams to ensure seamless transitions of care.
Monitor patient outcomes and implement improvements based on data-driven insights.
Manage departmental budget and resources effectively.
Ensure compliance with regulatory requirements and accreditation standards.
Stay current with industry trends and best practices to drive innovation.

What You Bring:

Active RN license in the state.
5 years of case management experience in a healthcare setting.
Strong communication and leadership skills to foster a collaborative work environment.

Preferred Qualifications:

Master's degree in Nursing or a related field.
3-5 years of leadership experience in case management.
Formal case management training for an advanced skillset.
