Inpatient Social Work Case Manager – LMSW/LCSW-C

Location: Baltimore, MD

Virtus Healthcare Staffing is seeking a dedicated Inpatient Social Work Case Manager to join our partner clinic in Baltimore, MD. We are looking for a compassionate and experienced Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker-Clinical (LCSW-C) to provide essential support and advocacy to patients and families in the general medical inpatient setting.

Responsibilities:

• Discharge Planning: Collaborate with the clinical team and medical provider to develop and implement comprehensive discharge plans, ensuring a smooth transition for patients from hospital to home or other appropriate care settings.
• Patient and Family Advocacy: Advocate for patients’ rights and needs, providing support and guidance to navigate complex healthcare systems and access available resources.
• Psychosocial Intervention: Assess and address patients’ psychosocial needs, offering counseling, crisis intervention, and emotional support to individuals and families facing challenging health situations.
• Resource Coordination: Connect patients and families with community resources, support services, and post-acute care options to ensure continuity of care and promote successful recovery.

Qualifications:

• Education: Master’s Degree in Social Work.
• Licensure: Active LMSW or LCSW-C license in the state of Maryland.
• Experience: Prior experience with discharge planning in an inpatient/hospital setting is required. Experience working with a general medical inpatient population is essential. This is NOT a mental health/psych position.
