Virtus Healthcare Staffing is searching for a dedicated and skilled Lead Palliative Care Physician to join our partner, a dynamic medical center in Jackson, MS. As the Lead Palliative Care Physician, you will be responsible for building and managing a patient panel while driving both clinical and financial Key Performance Indicators (KPIs). You will work autonomously with a Monday-through-Friday schedule, conducting in-home visits to a panel of patients in a value-based care model. Your role is crucial in improving the quality of life for complex patients and in fostering strong relationships with local healthcare providers.
Clinical Care & Patient Management: Proactively identify patients who qualify for palliative care services and develop a patient panel. You will treat patients in their homes to drive healthcare outcomes, evaluating and treating symptoms that may interfere with quality of life, such as pain, anxiety, and insomnia.
Goals of Care: Conduct goals of care and family meetings as needed, and document patient and family wishes in state-specific forms. You will assist with hospice transitions as appropriate.
Leadership & Collaboration: Educate and support the care team around palliative care, hospice care, and advanced directives. You will collaborate with a multidisciplinary team (IDT) and develop key relationships with local providers to improve market alignment.
Strategic & Administrative Duties: You will be responsible for the outcomes of your patient panel, developing and presenting metrics to executive leadership, and participating in monthly meetings to review KPIs.
Required:
A Doctorate Degree in Medicine.
Board Certified in Hospice and Palliative Medicine.
A current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare.
Comfort and skill in advance care planning and in goals of care discussions.
Expertise in managing medication panels for complex patients.
Experience:
Experience with Medicare Advantage, managed Medicaid populations, and commercial populations.
Experience integrating acute and behavioral health care and an appreciation of social determinants of health is required.
Additional Requirements:
Must be willing and able to obtain hospital privileges at required facilities.
Willingness to become licensed in multiple states as company and patient needs require.
Must have reliable transportation.
