Licensed Clinical Social Worker (LCSW) – Case Management

Location: Joshua Tree, CA

Virtus Healthcare Staffing is seeking a dedicated LCSW to join our partner hospital’s Case Management team in Joshua Tree, CA. This role focuses on facilitating patient care transitions, optimizing resource utilization, and ensuring patient well-being throughout their healthcare journey.

Responsibilities:

• Transition Management: Assess and address patients’ needs for smooth transitions across the care continuum, promoting appropriate length of stay, preventing readmissions, and enhancing patient satisfaction.
• Care Coordination: Collaborate with the healthcare team to ensure efficient throughput and timely delivery of care at the appropriate level, prioritizing patient safety and quality.
• Psychosocial Assessment and Intervention: Conduct complex psychosocial assessments and provide interventions to support patients and families facing emotional, social, and practical challenges related to their health condition.
• Compliance: Adhere to state and federal regulations, TJC accreditation standards, and hospital policies related to case management and patient care.
• Education: Provide education to physicians, patients, families, and caregivers on available resources, discharge planning, and self-management strategies.
• Mentorship: Precept and guide new staff members as a resource and role model.
• Quality Improvement: Actively participate in department quality improvement initiatives and contribute to committees or projects as assigned.

Qualifications:

• Required:
  • Master’s Degree in Social Work
  • Active California LCSW license
• Preferred:
  • Accredited Case Manager (ACM) certification.
  • A minimum of two years of acute hospital experience.
• Skills:
  • Demonstrated organizational skills.
  • Excellent verbal and written communication skills.
  • Ability to lead and coordinate in a fast-paced environment.
  • Critical thinking and problem-solving skills.
  • Computer literacy.

• Full-Time
• 8:00 AM – 4:30 PM, Weekdays and every other weekend
