Virtus Healthcare Staffing seeks a dedicated and experienced healthcare professional | US Applicants Only to join our partner, a 320-bed, faith-based, non-profit acute care hospital in Fullerton, CA, as a Manager of Care Management.

Lead and Innovate in Care Management | US Applicants Only

As a Manager of Care Management, you will oversee the daily operations of the Care Management department, providing leadership to a team of care managers and ensuring effective care coordination for patients in a Level II Trauma Center with a 24/7 Emergency Department.

Responsibilities:

• Leadership & Management: Oversee daily operations, supervise and mentor staff, ensure compliance, manage budgets and resources.
• Patient Care: Collaborate with interdisciplinary teams, develop and implement comprehensive care plans.
• Quality Improvement: Monitor patient outcomes, identify areas for improvement, and implement best practices.
• Communication: Act as a liaison between patients, families, and healthcare providers.

Qualifications:

• Education: Bachelor's Degree in Nursing (BSN) or Master's degree in Social Work (MSW).
• Experience: Three (3) years of experience in an ambulatory or acute care setting.
• Skills: Strong project management and operations experience.

Preferred Qualifications:

• Master's degree in Nursing (MSN) or Health Administration (MHA).
• Case Management Certification (CCM or ACM) upon hire.
• Three (3) years of management experience in a healthcare setting.
