This is an exciting opportunity to provide comprehensive care to a diverse senior patient population and contribute to a collaborative and innovative healthcare team.

Deliver Comprehensive Senior Care in a Collaborative Setting

As a Nurse Practitioner/Primary Care Provider, you will perform patient assessments, develop care plans, coordinate care, and contribute to quality improvement initiatives.

Responsibilities:

• Patient Assessments: Perform comprehensive assessments, including initial, semi-annual, and unscheduled visits.
• Care Planning: Collaborate with physicians and the interdisciplinary team (IDT) to develop individualized care plans.
• Discharge Planning: Work with physicians and the IDT to facilitate smooth transitions from care settings.
• Nursing Home Care: Maintain and oversee care for participants in nursing homes.
• Care Coordination: Collaborate with providers and the IDT to ensure effective care coordination.
• Quality Improvement: Review data to identify areas for improvement and contribute to best practices.
• On-Call Coverage: Participate in a rotating on-call schedule.

Qualifications:

• Education: Graduation from an accredited School of Nursing.
• Certification: Certified as a Nurse Practitioner with Prescriptive Authority.
• Licensure: Active Nurse Practitioner State License.
• Experience: Minimum of two years of experience working with the senior population.
• Knowledge: Current knowledge of chronic care/geriatric issues and best practices.
• Other: Valid State Driver's License with a clean DMV record and proof of vehicle insurance.
