Virtus Healthcare Staffing is seeking a compassionate and skilled Physical Therapist (PT) to join our partner, a medical facility located in San Bernardino, CA. This role offers a rewarding opportunity to work as a key member of an Interdisciplinary Team (IDT), providing vital restorative and rehabilitative services to a frail or elderly population in both facility-based and home settings.
Position Overview:
The Physical Therapist plays an integral role within the IDT, collaborating closely with Primary Care Physicians, the rehabilitation team, and other clinical staff to develop and implement comprehensive care plans. The focus is on coordinating and providing evidence-based Physical Therapy services aimed at maximizing participant independence and safety, restoring function, and improving overall quality of life. This position involves conducting assessments, delivering interventions, supervising support staff, and providing education within both the center and participants' homes.
Key Responsibilities:
Conduct comprehensive Physical Therapy assessments, both in the facility and participants' homes, to evaluate needs, establish preferences, and set functional goals.
Actively participate in Interdisciplinary Team (IDT) meetings, contributing clinical expertise to the development of individualized participant care plans.
Deliver and meticulously document Physical Therapy interventions based on physician orders and established care plans, including:
Active and passive exercises, muscle re-education, gait training, and functional training for Activities of Daily Living (ADLs).
Therapeutic modalities such as massage, heat, sound, water, light, or electricity as appropriate.
Supervise Therapy Assistants, providing necessary training, instruction, and guidance, particularly for the implementation of Functional Maintenance Programs (FMP).
Provide clear training and instructions to staff, participants, and their caregivers/families on the proper use, care, and maintenance of therapeutic equipment.
Collaborate effectively with all members of the IDT to ensure coordinated and holistic participant care.
Minimum Qualifications:
Education:
Bachelor's Degree in Physical Therapy from an accredited program PLUS a minimum of five (5) years of experience as a Physical Therapist required.
(Master's Degree in Physical Therapy or Doctorate in Physical Therapy (DPT) is preferred).
(Qualifying physical therapy degrees earned outside the United States must be evaluated to confirm U.S. equivalency).
Licensure:
Must possess a valid California Physical Therapy License, or provide proof of license eligibility in California.
Experience:
Two (2) years of Physical Therapy experience working in a clinical setting with a frail or elderly population is preferred.
Certification:
Current CPR certification is preferred.
Work Environment:
This role involves working in a dynamic, team-based environment within a clinical center and providing care directly in participants' homes.
Focus is on serving a geriatric or frail adult population requiring restorative and rehabilitative care.
