Maximize Independence and Function for Older Adults: Physical Therapist Opportunity in Stockton
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our partner, a leading healthcare facility in Stockton, California. In this role, you will provide essential physical therapy services to participants in a center-based and home-based setting, helping them achieve their highest level of physical function and well-being.
Enhance the Lives of Older Adults Through Expert Physical Therapy
As a Physical Therapist, you will provide assessments, develop care plans, and deliver therapeutic interventions to maximize independence and function for older adults.
Responsibilities:
- Assessment and Care Planning:
- Conduct comprehensive physical therapy assessments in both the facility and participants' homes to identify their needs, preferences, and goals.
- Collaborate with the interdisciplinary team (IDT), including the Primary Care Physician, rehabilitation team, and other members, to develop individualized care plans that promote independence and safety.
- Therapeutic Interventions:
- Deliver evidence-based physical therapy interventions in accordance with physician orders and care plans.
- Provide a range of treatments, including:
- Active and passive exercises.
- Muscle re-education.
- Gait and balance training.
- Functional training for activities of daily living (ADLs).
- Modalities such as massage, heat, ultrasound, and electrical stimulation.
- Supervision and Training:
- Supervise and mentor Therapy Assistants, providing guidance and training on implementing Functional Maintenance Programs (FMPs).
- Educate staff, participants, and caregivers/families on the proper use and care of therapeutic equipment.
- Collaboration and Communication:
- Communicate effectively with participants, families, and the IDT to ensure coordinated and patient-centered care.
- Document all assessments, interventions, and progress in a timely and accurate manner.
Qualifications:
- Education and Experience:
- Bachelor's Degree in Physical Therapy with five (5) years of experience.
- Master's Degree in Physical Therapy or Doctorate in Physical Therapy preferred.
- Two (2) years of experience working with a frail or elderly population in a clinical setting preferred.
- Licenses and Certifications:
- Valid California Physical Therapy License or proof of license eligibility.
- If the degree was earned outside the US, it must be evaluated for US equivalency.
- Current CPR certification preferred.
- Skills and Knowledge:
- Strong clinical skills in geriatric physical therapy.
- Knowledge of various treatment modalities and therapeutic exercises.
- Excellent communication, interpersonal, and teamwork skills.
- Ability to work independently and as part of an interdisciplinary team.
