Virtus Healthcare Staffing is seeking a dedicated Primary Care Physician to join our partner medical facility in Fresno, CA, within their Program of All-Inclusive Care for the Elderly (PACE). This is an exciting opportunity to provide comprehensive care to a diverse patient population and contribute to a collaborative and innovative healthcare team.

Provide Comprehensive Primary Care within a PACE Program in Fresno, CA

As a Primary Care Physician within the PACE program, you will conduct patient assessments, develop care plans, and provide essential medical care to frail or elderly participants.

Responsibilities:

• Patient Assessments: Perform in-person comprehensive assessments and provide continuous medical coverage to participants.
• Care Planning: Collaborate with the interdisciplinary team (IDT) to develop individualized care plans for each participant.
• Discharge Planning: Work with community-based physicians and the IDT to facilitate smooth transitions for participants discharged from acute or long-term care settings.
• Nursing Home Care: Help maintain and oversee the care of participants residing in nursing homes, conducting regular visits as needed.
• Care Coordination: Collaborate with physicians, community-based providers, and the IDT to ensure effective care coordination.
• Leadership: Provide direction and support to Nurse Practitioners and other members of the IDT.
• On-Call Coverage: Participate in a rotating on-call schedule, providing coverage during nights and weekends.

Qualifications:

• Education: Doctorate Degree in Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
• Board Certification: Internal Medicine, Family Practice, or Geriatrics (preferred).
• Licensure: Active California Physician License without restrictions.
• Other: Active state CDS Certification and DEA.
• Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants.
