Virtus Healthcare Staffing seeks a dedicated Registered Nurse | US Applicants Only to join our partner, a 204-bed acute care hospital in Tarzana, CA, as a Care Manager. This full-time position offers an exceptional opportunity to utilize your clinical expertise and care coordination skills in a Level I Trauma Center. The hospital is known for its newly expanded, ultra-modern Emergency Department.

Provide Expert Care Coordination in a Leading Trauma Center | US Applicants Only

As a Care Manager, you will play a crucial role in coordinating patient care, ensuring timely and appropriate services, and advocating for patient needs within a Level I Trauma Center.

Responsibilities:

• Assess patients' needs and develop individualized care plans to ensure timely and appropriate care delivery.
• Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to coordinate patient care and facilitate smooth transitions.
• Monitor patients' progress and intervene as needed to address any challenges or barriers to care.
• Educate patients and families about their medical conditions, treatment options, and available resources.
• Advocate for patients' rights and needs, ensuring they receive high-quality, patient-centered care.
• Manage utilization of healthcare resources effectively, promoting cost-effective care without compromising quality.
• Maintain accurate and timely documentation, adhering to hospital policies and procedures.
• Participate in quality improvement initiatives and contribute to the enhancement of care management processes.
• Stay abreast of current healthcare trends and regulations, ensuring compliance and best practices in care management.

Qualifications:

• Education: Bachelor's Degree in Nursing or equivalent education/experience.
• Licensure: California Registered Nurse License upon hire.
• California Fire and Life Safety Card – National Organization within 30 days of hire. Experience:
  • Two (2) years of experience in a healthcare-related field (acute, ambulatory, post-acute, etc.).
  • Two (2) years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager.
• RNs employed prior to 8/2021 are encouraged to obtain a Bachelor's degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment.

Preferred Qualifications:

• Master's Degree in Nursing or a healthcare-related field.
• Upon hire: Certification in Case Management (ACM or CCM).
