Virtus Healthcare Staffing seeks a dedicated RN Care Manager to join our partner, a renowned acute care hospital in Santa Monica, CA. This is an excellent opportunity for a compassionate and experienced RN to play a vital role in coordinating patient care and ensuring seamless transitions.
Coordinate and Advocate for Comprehensive Patient Care
As an RN Care Manager, you will be responsible for coordinating patient care across the continuum, ensuring effective utilization of resources, and advocating for optimal patient outcomes. You will work closely with patients, families, and the interdisciplinary team to develop and implement individualized care plans.
Responsibilities:
Assess patient needs and develop comprehensive care plans.
Coordinate patient care across the continuum, including inpatient, outpatient, and community resources.
Advocate for patients and families, ensuring their needs are met.
Collaborate with physicians, nurses, and other healthcare professionals to facilitate effective care transitions.
Monitor patient progress and adjust care plans as needed.
Provide education and support to patients and families.
Ensure compliance with regulatory requirements and hospital policies.
Facilitate timely and appropriate discharge planning.
Participate in quality improvement initiatives.
Manage utilization of resources to ensure cost-effective care.
Required Qualifications:
Bachelor's Degree in Nursing or equivalent education/experience.
All Registered Nurses without a minimum of a Bachelor's degree in Nursing must obtain a Bachelor's degree in Nursing or higher within 3 years of hire.
RNs employed prior to 8/2021 are encouraged to obtain a Bachelor's degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment.
Upon hire: California Registered Nurse License.
2 years Experience in Healthcare related field (Acute, Ambulatory, Post-Acute, etc.).
2 years Experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager. TIP candidates must have experience in the same type of nursing unit in which the CM position is available.
Preferred Qualifications:
Master's Degree in Nursing or Healthcare related field.
Upon hire: Certification in Case Management (ACM or CCM).
