Virtus Healthcare Staffing seeks a dedicated Registered Nurse to join our partner, a 483-bed acute care facility in Orange, CA, as a Care Manager.

Provide Expert Care Coordination in a Magnet-Designated Hospital

As a Care Manager RN, you will assess patient needs, develop care plans, and collaborate with interdisciplinary teams to ensure high-quality, patient-centered care in a Level II Trauma Center.

Position Overview:

This full-time, day shift position offers an exceptional opportunity to utilize your clinical expertise and care coordination skills in a Magnet-designated hospital. The facility provides a wide range of services, including specialized programs in cancer treatment, kidney transplant, heart and vascular care, women's services, spine and neurology, and orthopedics.

Key Responsibilities:

• Patient Assessment & Planning: Assess needs, develop individualized care plans.
• Interdisciplinary Collaboration: Coordinate care with physicians, nurses, social workers.
• Patient Advocacy & Education: Educate patients/families, advocate for their rights.
• Resource Management: Manage utilization, promote cost-effective care.
• Documentation & Compliance: Maintain accurate records, adhere to policies.
• Quality Improvement: Participate in initiatives, enhance care processes.

Qualifications:

• Education: Graduate of an accredited school of nursing.
• Licensure: California Registered Nurse License (upon hire).
• Experience: Three (3) years clinical experience in an HMO, medical group, hospital, or clinic.

Preferred Qualifications:

• Bachelor's or Master's Degree in Nursing.
• Case Management Certification (CCM, ACM) (upon hire).
• Specialty Certifications (Oncology, Rehab, Pediatrics, Transplant, Wound Care) (upon hire).
• Three (3) years utilization or case management experience.
