Job Summary:
Virtus Healthcare Staffing is seeking a dedicated and compassionate Registered Nurse (RN) to join our partner hospital's Home Health team in West Reading, PA. In this role, you will provide professional nursing services to patients in their homes, managing their care based on physician orders and established care plans. As the primary case manager, you will oversee the care process, ensuring that patients receive personalized care and attention while following the Nurse Practice Act and corporate policies.
Key Responsibilities:
- Deliver professional nursing care to patients in their homes, adhering to physicians' orders and the individualized plan of care.
- Conduct thorough nursing assessments and develop, implement, and evaluate personalized care plans for each patient.
- Provide primary case management for assigned patients, overseeing all aspects of their home health care.
- Administer medications, treatments, and interventions as prescribed, monitoring patient progress and reporting any changes in condition to the appropriate healthcare providers.
- Collaborate with physicians, other nurses, therapists, and healthcare professionals to ensure a coordinated, interdisciplinary approach to patient care.
- Educate patients and their families on disease management, medication administration, and preventive care to promote optimal health outcomes.
- Maintain accurate and up-to-date documentation of all patient interactions, assessments, and interventions in the electronic medical record (EMR) system.
- Provide support and guidance to home health aides and other caregivers involved in the patient's care.
- Adhere to infection control and patient safety protocols to ensure a safe care environment for patients and staff.
- Participate in on-call rotations as required, providing emergency care and support when needed.
- Assist in discharge planning, coordinating resources for patients transitioning from home health care to other care settings or levels of care.
Education Requirements:
- Graduate of an accredited school of Nursing (Required)
Certifications & Licensures:
- RN license to practice in the state of Pennsylvania (Required)
- Basic Life Support (BLS) certification (Required)
- Valid Driver's License (Required)
- Current Automobile Insurance (Required)

- Clinical Nursing Expertise: Strong knowledge of nursing care in a home health setting.

Work Environment:
The RN – Home Health will work primarily in patients' homes, traveling throughout the assigned region to provide care. You must be able to manage time effectively, work independently, and demonstrate strong clinical judgment in the field. Occasional participation in on-call rotations will be required, as well as communication with the interdisciplinary team for coordinated care.
