Social Worker

Location: San Jose, CA

Virtus Healthcare Staffing is seeking a dedicated Social Worker to join our partner medical facility in San Jose, CA. In this role, you will work closely with patients, families, and the interdisciplinary team (IDT) to provide comprehensive support, resources, and care coordination.

Essential Job Duties:

• Psychosocial Assessments and Care Planning: Conduct thorough psychosocial assessments to understand patient needs, preferences, and goals. Actively participate in IDT meetings to develop individualized care plans.
• Social Work Interventions: Deliver and document social work interventions, including arranging necessary resources and services, assisting with care transitions, and providing individual and group counseling and case management.
• Care Transitions: Work collaboratively with the primary care physician and other care team members to facilitate smooth transitions between healthcare settings (e.g., hospitals, skilled nursing facilities, home).
• Care Conference Facilitation: Initiate, coordinate, and facilitate care conference meetings to ensure effective communication and collaboration among the care team, patients, and their support networks.
• Discharge Planning: Provide comprehensive discharge planning services for patients disenrolling from the program, ensuring a safe and supportive transition.

Job Requirements:

• Master’s Degree in Social Work (MSW)
• Minimum of one (1) year of experience working with a frail or elderly population
• Previous experience coordinating and facilitating care conference meetings
• Previous experience assisting people with behavioral health & substance abuse issues, preferred
